Congratulations on your acceptance, and welcome to the Idaho College of Osteopathic Medicine (ICOM). The following outlines the requirements necessary for matriculating to ICOM.

The non-refundable fee of $1,500 is credited toward your tuition upon matriculation.

ICOM respects and protects your privacy.


Official transcripts are required from all individual educational institutions showing completion of every post-secondary course and the awarding of every degree earned.  The transcripts included in your AACOMAS application will be used for this purpose.  If you are currently taking coursework you must send an official transcript showing the completion of the courses or degree conferred.  Transcripts must be sent directly from the educational institution to our Registrar. Transcripts that are hand-delivered or forwarded by the applicant/accepted student will not be accepted.

All incoming students are required to submit and pay directly to Certiphi for a criminal background check (CBC) and drug screening test. Instructions for the CBC and drug test will be made available on our matriculation website. The results from your CBC and drug screening test will be sent directly to you and the ICOM Office of Admissions via “certiphi.reportdelivery” when complete, therefore it is not necessary for you to forward your results to us. ICOM reserves the right to put restrictions on or rescind any offer of admission based on information received on your background check or drug screen.  Also note that should any charge occur after acceptance, you are required to inform ICOM within 24 hours of when the charge occurs. Failure to report such charges, whether guilty or not, can result in your offer of admission being rescinded. Upon review of the charge, ICOM reserves the right to rescind the offer of admission prior to matriculation.

All are required to use Certiphi for substance abuse screening. Please use the instructions below to request your screening.

Drug Screening Instructions

  1. Click the link or paste it into your browser:
  2. If this is your first time using the Application Station site then please click “Sign Up” to create an account. Once your account has been created please click “Log In”. If you already have an account then you can click “Log In” right away.
  3. Enter your Username and Password.
  4. Enter the Code: IDAHOCOMDS in the Application Station Code field.
  5. Follow the instructions on the Application Station web site to complete the application.

In addition to the screenings instructions outlined above ICOM requires the following to be completed for matriculation. ICOM has partnered with Certiphi to help with the management of the additional matriculation documentations.  

MyRecordTracker through Certiphi will be reaching out to you via email. All of ICOM’s matriculation forms can be accessed by setting up an account with MyRecordTracker including the Matriculation Agreement, Health and Immunizations forms, Technical Standards and the Academic Handbook.

Idaho College of Osteopathic Medicine students are required to provide a completed medical history form, proof of immunization, and a completed physical examination conducted by a licensed physician (using ICOM’s forms, available on the MyRecordTracker Website). Students are responsible for maintaining immunizations required by ICOM in order to complete all required supervised clinical practice experiences in the osteopathic medical program curriculum. All students must provide proof of adequate immunization for the following:

  • Annual TB test (two step-PPD if necessary)
  • Tetanus toxoid immunization
  • Hepatitis B virus (HBV)
  • Measles (Rubeola), Rubella and Mumps
  • Varicella
  • Influenza*


All Idaho College of Osteopathic Medicine (ICOM) students are required to have health insurance.  Students need to maintain health insurance all four years of medical school in order to attend early clinical classes and third/fourth year rotations within the clinics and hospital systems. Basic Life Support (BLS) Certification for Healthcare Providers through the American Heart Association is required before you begin classes at ICOM. It is highly recommended that you take a course near you well in advance of when classes start. Upload your BLS card to MyRecordTracker once it is complete.
